FY 2026 ICD-10-CM Updates/changes: Key Additions and Revisions from October 1, 2025

The Fiscal Year (FY) 2026 ICD-10-CM updates and changes includes substantial revisions that will impact clinical documentation and coding practices beginning October 1, 2025. The update introduces 487 new diagnosis codes, 38 revisions, and 28 deletions, reflecting ongoing efforts to enhance clinical specificity and capture emerging conditions and care contexts more accurately.

Enhanced Coding for Digestive System Symptoms and Abdominal Pain

To improve granularity in coding abdominal and pelvic pain, 16 new “R” codes have been added. These provide greater specificity for pain and tenderness in the pelvic, perineal, subpubic, abdominal, and flank regions. Notably, code R10.2- (Pelvic and perineal pain) has been reclassified as a parent code in 2026 ICD-10-CM updated codebook.

A significant addition is R11.16, created to identify Cannabis Hyperemesis Syndrome (CHS)—a condition involving nausea and vomiting in chronic cannabis users.

Additional enhancements include five new codes to capture specific types of costovertebral angle tenderness, as well as updates to immunological findings. R76.8- has been designated as a parent code, with R76.89 introduced to document elevated immunoglobulins, NOS (not otherwise specified).

Expanded “Z” Codes: Social Determinants of Health and Contextual Factors

FY 2026 ICD-10-CM changes adds several “Z” codes that support the documentation of social determinants of health (SDoH) and environmental exposures:

  • Z59.86 (Financial insecurity) has been updated to a parent code, with three new child codes added to specify financial hardship scenarios.

  • Three new Z77.3- codes are now available to report contact with or exposure to war zones.

Increased specificity has also been introduced for food allergies:

  • Z91.011 (Allergy to milk products) and Z91.012 (Allergy to eggs) have each been converted into parent codes with three additional sub-codes to capture more detailed allergic responses.

Oncology-Specific Code Updates – 2026 ICD-10-CM Updates/changes

Several changes have been made to enhance oncology coding:

  • C50.A-, denoting malignant inflammatory neoplasm of the breast, has become a parent code with three new codes to more precisely capture inflammatory breast cancer (IBC).

  • D71.- has been designated as a parent code for functional dimalignancysorders of polymorphonuclear neutrophils, with three new related codes introduced.

  • Z15.06-, a new parent code, documents genetic susceptibility to malignant neoplasms of the digestive system, including:

    • Z15.060 – Susceptibility to colorectal cancer

    • Z15.068 – Susceptibility to other digestive system cancers

Emergency Department Coding Enhancements- 2026 ICD-10-CM Updates/changes

For emergency department (ED) coding, new codes target specific anatomical injuries:

  • S30.11-: Contusion of abdominal wall

  • S30.12-: Contusion of groin

  • S30.13-: Contusion of flank

New codes also enable more accurate reporting of anaphylactic reactions triggered by food allergens such as milk and eggs, including:

  • T78.07-: Anaphylaxis from dairy with baked milk tolerance

  • T78.080-: Anaphylaxis from eggs with baked egg tolerance

Multiple Sclerosis and Neurological Conditions- 2026 ICD-10-CM Updates

Revisions have been made to G35.- (Multiple sclerosis), now a parent code, to facilitate reporting of MS subtypes such as relapsing-remitting and primary progressive MS.

New codes also address:

  • Congenital malformations

  • Usher syndrome

  • Genetic neurodevelopmental disorders, categorized under new parent code QA0.-

Ophthalmological Code Revisions – 2026 ICD-10-CM Updates/changes

Chapter 7 introduces 19 new codes and one new parent code for inflammations of the eyelid:

  • H01.8-: Parent code with laterality-specific subcodes, such as H01.81 for inflammation of the right upper eyelid.

Additionally, two new parent codes were added:

  • H05.83-: Thyroid orbitopathy, now with laterality options

  • H40.84-: Neovascular secondary angle closure glaucoma

Musculoskeletal System 2026 ICD-10-CM Updates/changes

Updates to Chapter 13 include:

  • M05.A: New code for abnormal rheumatoid factor and anti-citrullinated protein antibody with RA.

  • M21.159: Updated to include “hip” in the description of unspecified varus deformity.

  • M24.076: New code for loose body in unspecified toe joint(s).

  • M61.129: Now specifies myositis ossificans progressiva in either upper arm (right or left).

Genitourinary System Enhancements

Two new subcategories offer detailed classification of immune complex membranoproliferative glomerulonephritis (IC-MPGN):

  • N00.B1: Acute nephritic syndrome with idiopathic IC-MPGN

  • N00.B2: Acute nephritic syndrome with secondary IC-MPGN

  • N04.B1: Nephrotic syndrome with idiopathic IC-MPGN

  • N04.B2: Nephrotic syndrome with secondary IC-MPGN

Also added is N07.B, which captures hereditary nephropathy with APOL1-mediated kidney disease (AMKD).

New Codes for Fluoroquinolone Reactions

A new parent code, T36.A-, was introduced for adverse effects, poisoning, and underdosing of fluoroquinolone antibiotics. This includes codes for accidental and intentional poisoning, adverse reactions, and underdosing incidents.
